The ship's construction started on July 14, 1952, and it was launched on December 11, 1954. The Forrestal was acquired by the military on September 29, 1955, and officially commissioned on October 1, 1955. It was active for 38 years and 11 months until it was decommissioned and stricken on September 11, 1993.

USS Forrestal underwent several reclassifications during its service, starting as CVA-59 on October 1, 1952, becoming CV-59 on June 30, 1975, and finally designated as AVT-59 in February 1992.
